Licensed Social Worker - FT Hybrid Schedule

About the Role

Provides comprehensive social work services, including assessment, care planning, case management, and intervention for patients and their families. Acts as a liaison between healthcare providers and community resources to coordinate care, promote integration, and foster community relationships through outreach activities.

Responsibilities

• Conducts psychosocial assessments, develops, and evaluates individualized social work plans to address emotional, mental, and behavioral health needs.
• Performs case management, resource referral, advocacy, and mediates services to ensure appropriate patient placement and support.
• Assists in program development, implementation, and evaluation, participating in quality improvement and departmental activities.
• Conducts research, documents interventions according to standards and regulations, and maintains accurate clinical records.
• Engages in staff meetings, training, and orientation to ensure ongoing professional development.

Requirements

• Master’s degree in social work.
• Licensed Clinical Social Worker or Licensed Social Worker in Hawaii.
• Basic Life Support certification from American Heart Association.

Preferred Skills

• At least one year of social work experience.
• Experience working with medical diagnoses in healthcare settings.
• Strong networking abilities with community agencies.
