Licensed Social Worker - FT Hybrid Schedule

Job Summary Provides social work services including assessment, care planning, and case coordination to patients and their families. Offers consultation to healthcare staff and initiates social work care plans. Acts as a liaison between the organization and the community to integrate services, coordinate care, and foster relationships through public relations activities. Essential Responsibilities Conducts psychosocial assessments, develops and implements social work plans, and provides therapy for individuals, couples, families, and groups. Focuses on preventing psychosocial issues and evaluating practice effectiveness, aiming to enhance or restore social and personal functioning.Performs case management, resource coordination, and intervention services. Provides information, referrals, advocacy, and education, ensuring appropriate service placement.Contributes to program development, policy implementation, and quality improvement initiatives. Participates in performance assessments, clinical audits, and ongoing education to stay current in medical social work.Engages in research, documents findings per standards and regulations, and maintains accurate medical records.Participates in departmental meetings, training, and orientation of new staff. Basic Qualifications Education Master's degree in social work. License, Certification, Registration Licensed Clinical Social Worker (Hawaii) or Licensed Social Worker (Hawaii).Basic Life Support certification from the American Heart Association. Preferred Qualifications At least one year of social work experience.Healthcare experience with medical diagnoses.Strong networking skills with community agencies.
