Licensed Social Worker - FT Hybrid Schedule

About the Role

Provides comprehensive social work services including assessment, care planning, case management, and coordination to patients and families. Acts as a liaison between healthcare providers and the community to facilitate service integration, coordinate care, and strengthen relationships through public relations activities.

Responsibilities

• Conducts psychosocial assessments, develops and implements social work plans, and provides therapy for individuals, couples, families, and groups.
• Aims to prevent psychosocial dysfunction and enhance social functioning.
• Performs case management, resource referral, advocacy, and mediation to ensure appropriate patient placement and support.
• Assists in program development, evaluation, and quality improvement efforts.
• Participates in departmental activities, staff training, and research documentation.

Requirements

• Master's degree in social work.
• Valid licensure as a Clinical or Social Worker in Hawaii.
• Basic Life Support certification from the American Heart Association.

Qualifications

• Preferred: minimum one year of social work experience.
• Preferred: healthcare background with medical diagnoses.
• Preferred: community agency networking skills.
