About the role

Legend Oaks Northwest Houston offers proactive and advanced rehabilitation programs. They are seeking a Licensed Social Worker for Skilled Nursing Facilities.

Responsibilities

  • Plan, develop, organize, implement, evaluate, and direct social service programs at the facility in accordance with current existing federal, state, and local standards, as well as our established policies and procedures.
  • Maintain the medically related emotional and social needs of the resident on an individual basis.
  • Review department policies and procedures, at least annually, and participate in making recommended changes.
  • Develop and implement policies and procedures for the identification of medically related social and emotional needs of the resident.
  • Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
  • Assist in developing and implementing social care plans and resident assessments.
  • Demonstrate compassion and understanding when working with families and residents during and other interactions to obtain social history and background information and while planning individualized social service programs when possible.

Qualifications

  • Must have, as a minimum, a bachelor’s degree in social work or a bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology.
  • Must be a licensed Social Worker.
  • Must have, as a minimum, 1 year of supervised social work experience in a skilled nursing/long term care setting working directly with individuals.
  • Experience in Skilled Nursing or Long Term Care is a plus.
  • Must demonstrate exceptional interpersonal and communication skills.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
  • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning.
  • Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing social service.
