Licensed Social Worker Assistant Director
About the role
The Assistant Director of Social Services oversees the Social Service department at CareOne at Somerset Valley. Key responsibilities include planning and directing social service programs, providing therapeutic interventions, coordinating interdisciplinary care planning, and ensuring compliance with policies and procedures.
Responsibilities
- Plan and direct a comprehensive social service program
- Participate in interdisciplinary team approach
- Provide therapeutic intervention and counseling
- Develop and document resident's plan of care
- Coordinate interdisciplinary care planning conferences
- Ensure resident and family input in care planning
- Conduct family meetings and discharge planning
- Facilitate safety and adhere to fire and sanitation codes
- Coordinate admissions and develop relationships with referral sources
- Verify insurance coverage and conduct facility tours
Requirements
- Masters Degree in Social Work or a related field
- Current license as a Licensed Social Worker (LSW) or Certified Social Worker (CSW)
- 3+ years of Social Work experience
- Experience in long-term care/skilled nursing preferred
Qualifications
Master's Degree in Social Work or a human services field, and a current license as a Licensed Social Worker (LSW) or Certified Social Worker (CSW) within the state of operations.
Skills
Strong interpersonal skills, excellent communication abilities, and the ability to manage multiple tasks and prioritize effectively.

$52,000 to $72,000 salary range
