About the role

This is a full-time, hybrid Licensed Social Worker (LSW) position in Ohio, requiring fieldwork and a commute to our FreedomCare office in Cleveland, OH at least two times a month. The role is essential to our Structured Family Caregiving (SFC) program, a caregiving arrangement where a patient lives with a principal caregiver who provides daily support based on the patient's needs. The goal is to provide necessary care while fostering the patient’s independence in a home environment, offering alternative long-term care options for those meeting nursing facility-level care needs.

Responsibilities

  • Interpret the philosophy and objectives of the agency to clients, families, and others.
  • Ensure compliance with HIPAA, Ohio Department of Medicaid and Ohio Department of Aging rules, provider policies, FreedomCare policies, and state/federal mandates.
  • Conduct initial in-person home assessments with the patient and caregiver to ensure compliance and clearly communicate the plan of care.
  • Provide day-to-day monitoring with monthly contact (telephonic and in-person) with both the patient and caregiver, including:
    • Patient enrollment (credentialing, claims operations, prior authorization appeals).
    • Patients’ satisfaction with care delivery and caregiver support, ensuring program integrity and triaging issues to appropriate internal staff.
  • Ensure quality of service aligns with the required plan of care by providing individualized coaching, continued education, and reviewing caregivers' goals to enhance competencies. In-person visits must occur at least every sixty calendar days.
  • Maintain detailed and accurate case records, documenting observations, changes, and notes in accordance with agency policy.
  • Act as the point of escalation and provide input to the client’s plan of care, including the physician’s plan of treatment, problem list, and implementing/monitoring changes.
  • Perform other job-related duties as required or directed by the supervisor.

Requirements

  • Valid and active Ohio Licensed Social Worker (LSW) license, issued by the CSWMFT Board.
  • Bachelor’s degree (BSW) or Master’s degree (MSW) in Social Work from a CSWE-accredited program.
  • Minimum of 2 years of clinical experience, with at least 1 year of experience with elders or adults with disabilities.
  • Home care experience is a plus.
  • Bilingual in Spanish is a plus.
  • Experience with CRM (Salesforce is a plus).
  • Valid driver’s license with an insured vehicle for travel within the state.

Pay

The base pay range for this role is $28.00–$33.00 per hour at the commencement of your first year of employment. Compensation decisions depend on factors such as skill set, years of relevant experience, education, location, and licensure/certifications.
