Licensed Social Worker - Ambulatory

Job Summary Provides comprehensive social work services including assessment, care planning, case coordination, and consultation to patients and families. Acts as a liaison between healthcare providers and the community to integrate services, coordinate care, and strengthen relationships through outreach activities. Essential Responsibilities Conducts assessments and develops social work care plans, addressing psychosocial needs, emotional, mental, and behavioral health, to improve or restore personal and social functioning.Performs case management, resource referral, advocacy, and intervention to support individuals, families, and groups in managing personal, social, or economic challenges.Assists in program development, implementation, evaluation, and quality improvement initiatives, ensuring compliance with standards and maintaining documentation.Engages in research, data analysis, and social policy evaluation, documenting findings per regulations.Participates in departmental meetings, staff training, and orientation activities. Qualifications & Skills Master’s degree in social work.Licensed Clinical Social Worker or Licensed Social Worker in Hawaii.Basic Life Support certification.Preferred: 1+ year of social work experience, healthcare background, and community agency networking skills.
