About the Role
Provides comprehensive social work services, including assessment, care planning, case management, and coordination for patients and families. Acts as a liaison between healthcare providers and the community to improve service integration and relationships through public relations activities.
Responsibilities
- Conducts psychosocial assessments, develops and evaluates social work care plans, and provides therapy for individuals, couples, families, and groups.
- Performs case management, resource referral, advocacy, mediation, and ensures appropriate patient placement.
- Contributes to program development, implementation, evaluation, and quality improvement initiatives.
- Engages in research, documentation, and compliance with regulatory standards.
- Participates in departmental meetings, training, and orientation activities.
Qualifications
- Master’s degree in social work.
- Licensed Clinical Social Worker or Licensed Social Worker in Hawaii.
- Basic Life Support certification from American Heart Association.
Skills
Preferred: Minimum one year of social work experience, healthcare experience with medical diagnoses, and strong community networking skills.
