Licensed Social Worker - Ambulatory

About the Role

Provides social work services including assessment, care planning, and case coordination to patients and families. Offers consultation to healthcare staff and providers. Acts as a liaison between healthcare and community resources to coordinate care, promote services, and strengthen relationships through outreach activities.

Responsibilities

• Prepares and evaluates assessments; develops and implements social work plans.
• Provides psychosocial evaluations, diagnoses, and treatment for individuals, families, and groups.
• Focuses on preventing psychosocial issues and improving social and emotional functioning.
• Manages cases, coordinates services, and monitors progress.
• Offers resource referral, advocacy, and education.
• Ensures appropriate patient placement and service delivery.
• Supports program development, implementation, and evaluation.
• Participates in quality improvement, research, and departmental activities.
• Maintains documentation and records compliance with regulations.
• Engages in staff meetings, training, and orientation duties.

Requirements

• Master’s in social work.
• Licensed Clinical Social Worker or Licensed Social Worker in Hawaii.
• Basic Life Support certification required.

Preferred Qualifications

• At least one year of social work experience.
• Healthcare background.
• Community networking skills.
