About the role
We are looking for an experienced LPN / Resident Care Coordinator (RCC) to join our clinical leadership team in a hands-on role.
Responsibilities
- Providing direct nursing care to residents in Assisted Living and Memory Care
- Administering medications and treatments per physician orders and regulations
- Coordinating resident care plans, assessments, and documentation as RCC
- Supervising and supporting Caregivers/QMAPs during shifts
- Assisting on the floor with resident care and staffing support
- Communicating effectively with families, providers, and leadership
- Ensuring compliance with health regulations and participating in audits and quality assurance
- Responding to clinical concerns, incidents, and emergencies
Requirements
- Current Colorado LPN license in good standing
- Experience in Assisted Living, Memory Care, or Long-Term Care preferred
- Strong medication management, clinical judgment, and documentation skills
- Willingness to assist with hands-on resident care
- Leadership or supervisory experience preferred
- Ability to stay organized, calm, and decisive in a fast-paced environment
