About the role
The Resident Care Coordinator (RCC) plays an integral role in the success of the team, the community, and creating a place residents are proud to call home. The primary purpose of this position is to collaborate with the Director of Nursing to plan, organize, develop, and direct the overall functions of resident care services in accordance with current federal, state, and local standards, guidelines, and regulations.
Responsibilities
- Management Functions:
- Assist to develop, maintain, update, communicate and enforce facility and state-specific written policies and procedures governing daily functions of the nursing service department, including the safety program.
- Maintain a current reference of written nursing material to meet the day-to-day needs of residents.
- Communicate nursing services schedules with other services to ensure continuity of residents’ total regimen of care.
- Assist to develop, implement, and maintain an ongoing Continuous Quality Improvement and Quality Assurance program for the nursing services department.
- Recommend to the Director of Nursing the equipment needs of the nursing department.
- Participate with the Director of Nursing to prepare and plan the nursing service department’s budget and staffing plan.
- Nursing Care Functions:
- Conduct daily nursing functions in accordance with current rules, regulations, and guidelines governing the long-term care facility.
- Ensure all nursing personnel comply with written policies and procedures, including the safety program.
- Admit, transfer, and discharge residents with documentation regarding care and services provided.
- Participate in the orientation of new residents/family members to the facility.
- Communicate with healthcare providers in a timely and appropriate manner.
- Utilize nursing personnel qualifications according to the resident’s care plan, wishes, and healthcare provider orders.
- Complete daily clinical and environmental rounds to evaluate resident care and report significant events.
- Meet and resolve issues with residents and/or family members; report significant issues to the supervisor.
- Participate in facility surveys (inspections) as requested by management.
- Follow clinical programs including risk management, infection prevention, quality, and staff development.
- Communication/Documentation Functions:
- Complete an initial assessment and develop a service plan for assigned residents.
- Update, review, and transcribe the service plan based on nursing needs and new healthcare provider orders.
- Communicate with healthcare providers to review treatment plans, complete orders, and progress notes per policy.
- Review residents’ health records for treatments, medication orders, diets, labs, etc., for implementation and follow-up.
- Requisition and arrange for diagnostic and therapeutic services as ordered and per established procedures.
- Communicate department policies and procedures to personnel, residents, and visitors.
- Provide written (24-hour report) and oral reports/recommendations regarding changes in resident condition and shift activities to nursing management.
- Fill out and complete Risk Watch for accidents/incidents involving residents or staff and submit to the Director of Nursing.
- Complete and file assessments and all required health record documentation.
- Report discrepancies in physician’s orders, diet changes, charting errors, etc., to the Director of Nursing.
- Complete clinically accurate documentation according to policy and procedures.
- Review resident health documentation and progress notes from the interdisciplinary team to determine if the service plan is being followed.
- Inform nursing personnel of new admissions and care needs; ensure rooms are ready for new admissions.
- Greet newly admitted residents upon arrival.
- Notify the resident’s attending healthcare practitioner and family when there is a change in the resident’s condition.
- Meet with assigned support personnel (Housekeeping, Dining Services, Caregivers, etc.) to plan shift services, programs, and activities and monitor task completion.
- Coordinate services with other interdisciplinary team members to ensure the resident’s total regimen of care is maintained.
- Assist in arranging transportation for residents and other third-party services.
- Medication Administration Functions:
- Review medication administration records for completeness and accuracy; report concerns to supervisor.
- Dispose of drugs and narcotics as required and per established procedures.
- Personnel Functions:
- Assist in the recruitment and selection of nursing service personnel.
- Assist to develop work assignments, schedule duty hours, and supervise nursing staff to ensure acceptable nursing standards.
- Assist in staff development; formally and informally monitor performance and provide coaching, counseling, and discipline as needed.
- Collaborate with the Director of Nursing and Executive Director to resolve staff performance issues, including those leading to termination.
- Complete timely performance evaluations as directed.
- Review complaints and grievances, complete investigations, and work with the Director of Nursing on resolution plans.
- Participate in planning, conducting, and scheduling in-service and orientation training classes meeting state and federal requirements.

- Capable of working with ill, disabled, elderly, and emotionally upset people within the facility.

- Proficiency in Microsoft Office Suite.
Education and Experience
- Minimum of two (2) years of nursing experience required.
- Minimum of one (1) year of leadership or care coordination experience preferred.
- Current, active nursing license in the applicable state, in good standing, required.
- Experience in a long-term care, assisted living, or similar healthcare setting preferred.
- Must meet all applicable federal, state, and local requirements for the position.
Supervisory Responsibility
This position has direct reports and supervisory requirements.

- Physical Requirements:
- Prolonged periods of standing.
- Ability to lift fifty (50) pounds.
- Moving, lifting, or transferring residents may involve lifting up to 75 pounds.
- Ability to exert maximum muscle force to lift, push, pull, or carry patients.
- Ability to use abdominal and lower back muscles over time without fatigue.
