Licensed Practical Nurse/PRN, Shady Oaks West

About the role

LSSI is hiring an LPN (Licensed Practical Nurse) to provide direct nursing services to clients in substance abuse programs. This role requires flexibility and availability on Mondays, with the option to work a full 12-hour shift.

Responsibilities

• Administer medications as prescribed.
• Assess, plan, and evaluate nursing care in the facilities.
• Identify changes in clients' conditions and take appropriate action.
• Plan and provide direct care that maximizes functional capacities.
• Complete documentation in accordance with regulations and facility practices.
• Demonstrate professional, positive behavior and carry out responsibilities with integrity.

Requirements

• High School Diploma
• Valid IL LPN license
• BLS/AED CRP trained
• At least 5 years’ experience in long-term care nursing
• At least 5 years’ experience with individuals with developmental disabilities
• Ability to provide leadership and training to direct care personnel
• Participate in ongoing learning opportunities to keep skills current
• Valid driver’s license, in good standing for the state of residency required
• Access to reliable transportation required

Qualifications
