Licensed Practical Nurse / LPN

About the role
As a Licensed Practical Nurse (LPN), you will provide compassionate, high-quality nursing care to residents in a supportive assisted living environment. Working alongside an interdisciplinary team, you will help promote residents' health, safety, and independence while ensuring care is delivered in accordance with clinical standards and organizational policies. Schedule
- Full-Time (40 hours per week)
- Monday–Friday, 11:30 AM – 8:00 PM
- Direct Hire
Pay
- $40.00 per hour
- $5,000 Sign-On Bonus
Responsibilities
- Provide skilled nursing care and medication administration to assisted living residents.
- Monitor residents' health status, document assessments, and communicate changes to the interdisciplinary care team.
- Collaborate with physicians, caregivers, and staff to ensure high-quality, resident-centered care.
- Maintain accurate clinical documentation and ensure compliance with regulatory and organizational standards.
- Educate residents and families on health-related needs while promoting resident safety and independence.
Requirements
- Graduate of an accredited Practical Nursing program.
- Active New York State Licensed Practical Nurse (LPN) license.
- CHHA experience preferred.
- Home care experience is a plus.
Skills
- Strong clinical assessment and patient care skills.
- Ability to work independently with minimal supervision.
- Excellent verbal, written, and interpersonal communication skills.
- Proficiency with Microsoft Outlook, Word, and basic office applications, including printing and scanning.
