Role Overview: The Licensed Practical/Vocational Nurse (LVN/LPN) provides nursing care, patient education, and assists physicians and RNs under supervision. Qualifications Graduated from an approved practical/vocational nursing programLicensed as a Practical Nurse in the practicing stateAt least one year of nursing experience Key Responsibilities Participate in planning and coordinating patient care with RNs, physicians, and policiesFollow individualized care plans and observe patient conditionsAccurately document patient status, care activities, and signs of changeAssist with activities of daily living and teach self-care techniquesMaintain a safe and supportive environment for patientsSupport physicians and RNs with complex procedures and treatmentsPrepare equipment, observe aseptic technique, and perform treatments within training scopeEngage in team meetings, training, and coordinate visits with other staffPerform treatments and procedures for which qualified, including documentation of competencyOther duties as assigned Organizational And Functional Requirements Report to the Director of Nursing ServicesOwn reliable transportation, valid driver’s license, and insuranceCommunicate effectively, perform physical duties, and travel to patient residences
