Licensed Practical Nurse - Primary Care

We are seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to patients under the supervision of a registered nurse or other designated supervisor. Responsibilities include collecting patient data through observationassisting in patient assessment and care planningdelivering care aligned with organizational policies and the patient care plancommunicating effectively with patients and team membersparticipating in quality improvement projects and team activities The ideal candidate will have graduation from an accredited practical nursing program or passed the LPN Boardsat least one year of recent LPN experience preferredexcellent communication skills and ability to work collaborativelyproficiency with electronic medical records and computer skillscurrent LPN license and BLS certification as required by the stateflexibility to adapt to unpredictable situations This role offers full-time hours with a schedule of four 10-hour days, primarily weekdays, with no weekend or call responsibilities.
