Licensed Practical Nurse - Primary Care

About the role

Provides nursing care with a focus on members/patients, building strong relationships and understanding their needs. Supports physicians and medical teams by preparing patients, communicating effectively, and performing routine and specialized nursing procedures within scope of practice. Ensures patient safety by adhering to safety, infection control, and medication administration protocols. Monitors patient conditions, reports changes, and responds to emergencies. Educates patients on health maintenance and post-care instructions. Records detailed patient information and assists in minor procedures and patient examinations.

Qualifications

• Graduation from an accredited Practical Nursing program and licensure as a Practical Nurse in Colorado.
• Must hold a valid Colorado Practical Nurse license and Basic Life Support certification.
• Demonstrates excellent customer service, teamwork, and communication skills.
• Additional certifications like IV therapy and BLS are preferred.
• Previous experience with diverse patient populations is advantageous.

Responsibilities

• Complies with safety and quality policies, supports clinical workflows, maintains infection control standards, and manages supplies.
• Assists in training new staff and performs administrative duties as needed.
• Upholds service principles to deliver culturally sensitive, high-quality care to members and colleagues.
