Licensed Practical Nurse - Primary Care

Role: Licensed Practical Nurse (LPN) provides patient care under supervision, participating as a team member to support patient assessment, care planning, and delivery. Responsibilities: Collect observations, communicate patient information, follow organizational policies, and participate in quality improvement projects. Deliver care based on patient care plans and principles of relationship-based care. Qualifications: Graduation from an accredited practical nursing program or passing the LPN Boards; recent LPN experience preferred. Strong communication skills, teamwork, computer proficiency, and flexibility are essential. Ability to work daytime hours and adapt to unpredictable situations. Licenses & Certifications: Valid LPN license and current BLS certification. Additional certifications may be required based on work area. Key Skills: Excellent communication, mentoring, instructing, and proficiency with electronic medical records.
