About the role
This LPN position is responsible and accountable for the delivery of nursing care to Veterans in the Primary Care Clinic located at the Lexington - Kentucky VA Health Care System. The LPN is an essential component of the PACT Initiative (Patient Aligned Care Teams).
Responsibilities
- Provide care to Veterans requiring complex practical nursing care.
- Demonstrate sufficient skill to resolve a range of nursing problems.
- Perform all duties within the scope of the LPN license, including medication administration.
- Prepare, administer, and document actions taken related to commonly prescribed medications.
- Recognize and appropriately respond to emergency situations and assist in resuscitation procedures.
- Recognize and appropriately respond to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies.
- Carry out support duties for complex diagnostic tests and specialized procedures.
Requirements
- Active - full - current and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State - Territory - or Commonwealth (i.e. - Puerto Rico) - of the United States or the District of Columbia.
- Knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems.
- Knowledge and skill sufficient to prepare, administer, and document actions taken specific to commonly prescribed medications.
- Recognize and appropriately respond to urgent or emergent patient care situations and initiate appropriate emergency interventions as directed.
- Recognize and understand human behavior and apply this knowledge to work effectively with patients, family members, and other staff.
- Carry out support duties for complex diagnostic tests and specialized practices or procedures.
Qualifications
- Graduate of an approved school of practical or vocational nursing.
- One year of successful practice as an LPN/LVN.
- Experience in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility.
Skills
- Ability to perform work that requires regular and recurrent walking, bending, lifting 45 pounds or more, stooping, reaching, stretching, and similar activities supporting various weights.
- Visual acuity, keen hearing, clear distinctive speech, and manual dexterity.
- Ability to work closely with others, respond to various alarms, and communicate verbally and in writing.
- Ability to care for Veterans with various medical and psychiatric issues, including those exhibiting unpredictable behavior.
- Exposure to communicable diseases and exposure to biohazard materials and sharps.

Pay
