Licensed Practical Nurse night Shift

About the role

The Center at CenterPlace is Greeley's premier, dedicated short-term rehabilitation facility featuring 54 private suites, state-of-the-art rehabilitation equipment, and care delivered in a hotel-like setting. Our center is seeking team-oriented, LPNs who are dedicated to patient outcomes and customer service.

Responsibilities

• Administers and documents the administrations of medications and treatments per physician orders
• Evaluates patients routinely and with changes of condition
• Communicates changes of condition, test results, etc. as per company policy
• Educates patients as necessary
• Safeguards the patient care plan is implemented
• Supervises and coordinates Certified Nursing Assistants (CNAs)
