Licensed Practical Nurse LPNs in Eastern CT
Clients throughout Eastern Connecticut requiring shifts from 6, 8, to 12 hours. Day and night shifts available. Work per diem, part-time, or around your current schedule. Shifts provide 1:1 care in the home environment.
Responsibilities
- Consult with management on complex/unique cases.
- Monitor/assess patients' condition and effects of the care plan.
- Oversee/supervise care provided to patients by Licensed Practical Nurses and paraprofessionals.
- Manage, plan, coordinate, and execute the patient's safe discharge from service.
- Ensure physician's orders, discharge summaries, and communication to team members, patients, and family members adhere to company and regulatory requirements.
- Communicate and consult with patient's physician as needed; manage and coordinate all aspects of assigned caseload.
- Assist patients with activities of daily living.
- Document content and process of all services provided in the internal system properly.
- Transport/utilize medical equipment to and from clients' homes/facilities.
- Travel to patients' homes and/or other facilities.
- Perform other related duties and participate in special projects as assigned.
Requirements
- CT LPN licensure.
- BLS, ACLS, or CPR certification.
- Minimum of one (1) year demonstrated experience as an LPN.
- Ability to put patients at ease and explain physician's instructions.
- Ability to multi-task with excellent organization skills.
- Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.
- Knowledge of medical terminology.
- Team player with the ability to work independently.
- Valid driver's license or NYS Non-Driver photo ID card (drivers preferred).
