How You'll Contribute A Licensed Practical provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice. Essential Functions Demonstrates knowledge of medications and their correct administration based on the age and clinical condition of the patientPerforms patient care considering needs specific to the standard of care for patient's age; includes geriatric and general inpatient rehabilitation patientsUnder the supervision of the RN, assists with the assessment on all patients and reassessments as per policy. Identifies and initiates appropriate rehabilitation nursing interventionsUnder the supervision of the RN, performs timely and accurate QI assessmentsCarry out the plan of care as indicated by the patient's needs and response to treatment; evaluates overall plan daily for effectiveness; updates interdisciplinary care plan accordingly based on changes in patient's condition and individual needsInteracts professionally with patient/family and involves patient/family in the formation of the plan of careDemonstrates ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain managementOther duties as assigned Additional Information Strong interpersonal and documentation skills required.Familiarity with rehab documentation systems is preferred. Additional requirements include: Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.Current state licensure as Licensed Practical Nurse.CPR/BCLS certification.
