Licensed Practical Nurse LPN / Registered Nurse RN – Full Time PM

Augusta Health and Rehabilitation is looking for a compassionate, dependable LPN or RN to join our nursing team on the PM shift at our small, community‑focused skilled nursing and assisted living campus in Augusta, WI.

Pay

• LPN: $25.50 per hour
• RN: $34.50 per hour

Schedule

Full‑time PM shift. Every other weekend and rotating holidays.

Responsibilities

• Provide direct nursing care and monitor residents' needs.
• Administer medications and treatments.
• Complete accurate and timely documentation.
• Communicate changes in condition to the appropriate members of the care team.
• Work collaboratively with CNAs and other nursing staff.
• Build positive relationships with residents and families.
• Help create a safe, respectful, and comfortable environment for the people who call AHR home.

Requirements

• Current Wisconsin LPN or RN license in good standing.
• Dependable and compassionate approach to resident care.
• Strong communication and teamwork skills.
• Ability to work independently while knowing when to ask for support.
• Commitment to treating residents and coworkers with dignity and respect.
• Nursing home experience preferred, but qualified candidates are encouraged to apply.
