About the Role

The Licensed Practical Nurse (LPN) plays an integral role in the success of our team, our community, and creating a place our residents are proud to call home. The primary purpose of this position is to provide high-quality, holistic nursing care in accordance with nursing practice standards and health care regulations governing professional nursing. The LPN leads or directs licensed and non-professional staff in delivering direct resident care and support functions.

Responsibilities

  • Nursing Care Functions
    • Conduct daily nursing functions in accordance with current rules, regulations, and guidelines for long-term care facilities/assisted living communities.
    • Ensure compliance with written policies and procedures, including the safety program, for all assigned nursing personnel.
    • Admit, transfer, and discharge residents with documentation of care and services provided.
    • Participate in orienting new residents and family members to the facility.
    • Make rounds with health care providers, Hospice staff, and Home Health staff as needed.
    • Utilize nursing personnel qualifications according to the resident's care plan, service plans, and health care provider orders.
  • Communication/Documentation Functions
    • Complete daily clinical and environmental rounds to evaluate resident care and report significant events.
    • Meet and resolve issues with residents and/or family members; report significant issues to the supervisor.
    • Participate in facility surveys (inspections) as requested by management.
    • Responsible for clinical programs as directed (e.g., dementia, wound care, quality assurance, infection control, admissions, and falls).
    • Complete initial assessments and assist in developing interim care plans/service plans.
    • Update, review, and transcribe care plans identifying goals, problems, approaches, and revisions based on nursing needs and health care provider orders.
    • Assist with service plans as directed by the Director of Nursing.
    • Communicate with health care providers to review treatment plans, complete orders, and progress notes per established policies.
    • Review resident medical records for treatments, medication orders, diets, labs, etc., for implementation and follow-up.
    • Requisition and arrange diagnostic and therapeutic services as ordered by health care providers.
    • Communicate department policies and procedures to personnel, residents, and visitors.
    • Provide electronic or written (24-hour report) and oral reports on resident condition changes and shift activities to nursing management.
    • Complete and submit accident/incident reports involving residents to the Director of Nursing.
    • Complete assessments and medical record documentation as required.
    • Provide resident and family information for care conferences or meetings.
    • Report discrepancies in physician orders, diet changes, or charting errors to the supervisor.
    • Ensure clinically accurate documentation according to policy.
    • Review interdisciplinary team progress notes to verify care plan adherence; report concerns to the supervisor.
    • Inform nursing personnel of new admissions and resident care needs; ensure rooms are ready for new admissions.
    • Greet newly admitted residents upon arrival.
    • Notify the resident’s health care practitioner and family of changes in the resident’s condition.
    • Assist in arranging transportation and third-party services for residents.
  • Medication Administration Functions
    • Prepare and administer medications as ordered by the health care provider.
    • Communicate with the pharmacy to ensure resident medications are available.
    • Maintain an adequate supply of floor stock medications, supplies, and equipment; report needs to the supervisor.
    • Review the medication administration record for completeness and accuracy; report concerns to the supervisor.
    • Dispose of drugs and narcotics according to established procedures.
  • Personnel Functions
    • Supervise the nursing unit to ensure personnel perform work assignments according to acceptable nursing standards.
    • Work with the Director of Nursing to resolve staff performance issues, including those leading to termination.
    • Report complaints and grievances to management and collaborate on action plans.
  • Safety and Sanitation Functions
    • Ensure protective equipment is available for staff.
    • Maintain clean and sanitary nursing service work areas, resident rooms, and treatment areas.
    • Recommend equipment and supply needs to the Director of Nursing.
    • Maintain stock levels of medications, medical supplies, and equipment to meet resident needs.
  • Perform other job-related duties as assigned.

Requirements

  • Must meet all applicable state and federal requirements for the position.
  • Must possess a current, unencumbered, active LPN/LVN license in the applicable state, in good standing throughout employment with no disciplinary action.
  • One year of experience in nursing in a home care, assisted living, or long-term care facility is preferred.

Supervisory Responsibility

This position ensures resident care is provided according to standards of practice and facility policies by CNAs/Caregivers, though CNAs/Caregivers do not directly report to the LPN.

    • Potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals.
  • Physical Requirements
    • Walk long distances and stand for extended periods.
    • Sustain repetitive movements (e.g., CPR compressions, bagging).
    • Maintain physical tolerance to stand, walk, or work on feet for 8–12 hours with minimal breaks.
    • Lift up to fifty pounds at times.
    • Push and pull up to fifty pounds at times.
  • Cognitive Requirements
    • Execute tasks independently.
    • Express yourself clearly and effectively.
    • General computer literacy.
  • Environmental Requirements
    • Exposure to ambient room temperatures and lighting.
  • May be required to work nights, weekends, and holidays.
