Responsibilities

  • Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
  • Aid residents with activities of daily living (ADLs) and ensure their comfort, dignity, and safety.
  • Supervise and direct nursing assistants and medication aides during the assigned shift.
  • Maintain close monitoring of residents' physical and emotional status and report any changes to the attending physician and responsible party.
  • Create, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
  • Complete accurate and timely clinical documentation according to facility policy and procedure.
  • Coordinate resident admissions, transfers, and discharges according to established procedures.
  • Involvement in quality assurance activities, staff education, and regulatory surveys as needed.
  • Perform other duties as assigned by the Director of Nursing or designee.

Requirements

  • Current, active LPN/LVN license in the state of practice.
  • CPR/BLS certification preferred (American Heart Association or American Red Cross).
  • Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
  • Experience in a supervisory or charge nurse role, preferred.
  • Strong clinical observation, medication administration, and documentation skills.
  • Knowledge of Federal and State long-term care regulations.
  • Effective verbal and written communication skills.
  • Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
  • Experience in a supervisory or charge nurse role.
  • Familiarity with electronic health records (EHR) systems.
  • Proficiency with Microsoft Office applications.

Qualifications

  • High school diploma or equivalent.
  • Completion of a licensed practical or vocational nursing program.
  • Passing scores on the NCLEX-PN or NCLEX-RN exam.

  • Ability to work independently and in a team environment.

Schedule
