Responsibilities
- Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
- Aid residents with activities of daily living (ADLs) and ensure their comfort, dignity, and safety.
- Supervise and direct nursing assistants and medication aides during the assigned shift.
- Maintain close monitoring of residents' physical and emotional status and report any changes to the attending physician and responsible party.
- Create, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
- Complete accurate and timely clinical documentation according to facility policy and procedure.
- Coordinate resident admissions, transfers, and discharges according to established procedures.
- Involvement in quality assurance activities, staff education, and regulatory surveys as needed.
- Perform other duties as assigned by the Director of Nursing or designee.
Requirements
- Current, active LPN/LVN license in the state of practice.
- CPR/BLS certification preferred (American Heart Association or American Red Cross).
- Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
- Experience in a supervisory or charge nurse role, preferred.
- Strong clinical observation, medication administration, and documentation skills.
- Knowledge of Federal and State long-term care regulations.
- Effective verbal and written communication skills.
- Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
- Experience in a supervisory or charge nurse role.
- Familiarity with electronic health records (EHR) systems.
- Proficiency with Microsoft Office applications.
Qualifications
- High school diploma or equivalent.
- Completion of a licensed practical or vocational nursing program.
- Passing scores on the NCLEX-PN or NCLEX-RN exam.
