Licensed Practical Nurse (LPN) PRN

About the role

We are seeking highly motivated and qualified Licensed Practical Nurses (LPN) (Per Diem) to join our nursing department.

Responsibilities

• Assess the health care needs of residents, make a nursing diagnosis, and collaborate with the nursing team to develop an interdisciplinary plan of care utilizing all available resources.
• Communicate the care plan effectively to all care providers to ensure continuity of care for residents.
• Establish and maintain functional communication with staff, other disciplines, residents, families, medical staff, and the community.
• Assess the effects of medications and treatments, monitor these effects, and provide appropriate feedback to the interdisciplinary team and medical managers to ensure residents receive individualized care.
• Use the Electronic Health Record (PCC preferred) to document and manage resident care.
• Lead and direct nursing staff in the safe and effective delivery of care by providing structure and guidance.
• Educate residents about their healthcare issues and care plan strategies.
• Apply knowledge of infection control, fire and disaster policies, and other facility, state, and federal regulations to maintain a safe environment for residents, staff, and the community.
• Uphold resident rights and treat residents, staff, and the community with respect and dignity.
• Handle resident and facility property and information with honesty, confidentiality, and respect.

Pay

Base pay: $42.00/hour

• Weekday Evening and Nights shift differential: $5.00 per hour
• Weekend Day shift differential: $5.50 per hour
• Weekend Evening and Nights shift differential: $7.00 per hour
