The Nursing & Rehabilitation Center is a 5-star, 460-bed facility providing services for short-term rehabilitation and long-term skilled nursing care, home care programs, ventilator dependent care, palliative and hospice care, a new Memory Care Unit and on-site dialysis. Lindner Residences assisted living community, and the proposed Fountaingate Gardens independent living community, to be built on the Commack campus. Responsibilities

• Overall responsibility for the Wellness Office in the absence of the Director of Resident Services.
• Supervise personal care services rendered by Gurwin Jewish: Fay J. Lindner Residences.
• Implement standards of care and policy and procedures for the Wellness Department.
• Coordinate and manage all Assisted Living Aides.
• Apprise family of changes in resident health and behavior.
• Responsible for control of medication and documentation of medication.
• Supervise daily shift logs to ensure compliance.

Requirements

• Current valid NYS LPN license with knowledge of geriatric nursing.
• Must adhere to the Department of Health Regulations as they pertain to Assisted Living Facilities.
• Previous supervisory experience.
• Must possess excellent organizational skills and be detail oriented.
• Ability to work independently in an active Assisted Living Community.
• Must have exceptional interpersonal skills, coupled with strong relationship building and be an effective leader.
