Licensed Practical Nurse (LPN) - Per Diem

Scope of Position

The Licensed Practical Nurse (LPN) provides care and directs the staff to follow the resident plan of care as determined by the Head Nurse. They supervise, assign, and evaluate the work performance of LNA's on the unit assigned, and identify any educational opportunities. They perform and sign drug counts at the change of shift, assess, identify, and report any changes in resident status, and assume responsibility for all aspects of care related to admissions, transfers, or discharges. They also follow all established safety and infection control policies/procedures and oversee all documentation/reporting in accordance with Rockingham County policy standards and State/Federal requirements.

Responsibilities

• Supervise, assign, and evaluate the work performance of LNA’s on the unit assigned

• Perform and sign drug counts at the change of shift

• Assume responsibility for all aspects of care related to admissions, transfers, or discharges
• Follow all established safety and infection control policies/procedures
• Oversee all documentation/reporting in accordance with Rockingham County policy standards and State/Federal requirements

Requirements

• Graduate of an accredited School of Nursing
• Must possess and maintain a current, valid, unencumbered New Hampshire LPN license
• Three (3) years of Long Term Care (or equivalent) experience preferred; or an equivalent combination of work and experience will be considered
• CPR / Basic Life Support (BLS) certified
• Knowledge of medical terminology and experience with electronic health records (EHR) systems

Qualifications

• Must have a valid driver’s license
• Must be able to lift up to 50 pounds
• Must be able to work flexible hours including weekends and holidays

• 2nd Shift: $4.00/hour
• 3rd Shift: $5.00/hour
• Weekend Premium: $2.00/hour
