About the Role
Provides professional nursing care, assessments, and evaluations to residents within their scope of practice.
Responsibilities
- Performs all licensed practical nursing functions under the supervision of a Registered Nurse.
- Completes resident assessments, develops and updates care plans, and implements appropriate interventions.
- Administers medications safely and accurately according to physician’s orders and facility policies, utilizing systems like Pyxis.
- Participates in care planning and ensures consistent, appropriate resident care.
- Maintains professional documentation and communicates effectively with the healthcare team.
- Guides staff to meet residents’ needs, practicing routines based on individual requirements.
- Participates in physician rounds, communicates changes in resident conditions, and transcribes orders per policy.
- Engages in ongoing education, shares knowledge, and assists with staff orientation.
- Handles incidents and complaints promptly, facilitating conflict resolution with respect for diversity.
Requirements
- Active LPN license in the practice state.
- Visual acuity and effective communication skills.
- Preferred less than 1 year of long-term care experience.
- Demonstrates commitment to mission, values, confidentiality, teamwork, and compliance with safety standards.
