Licensed Practical Nurse (LPN) | Long Term Care

We are seeking a Licensed Practical Nurse (LPN) to provide professional nursing care to residents in a healthcare facility. Key Responsibilities Include Performing all licensed practical nursing functions under the supervision of a Registered Nurse.Conducting resident assessments, implementing care plans, and prioritizing care needs.Administering medications safely and accurately following policies and physician orders, with proficiency in the Pyxis medication system.Participating in care planning, updating resident care plans, and ensuring compliance with documentation standards.Communicating effectively with residents, families, and healthcare team members, including participating in physician rounds and transcribing orders.Guiding staff to meet residents' needs, sharing knowledge, and assisting with staff orientation.Investigating incidents and complaints, facilitating conflict resolution, and respecting diversity. Qualifications & Skills Active LPN license in the state of practice.Visual, communication, hearing, and comprehension abilities necessary to perform job functions.Commitment to confidentiality, teamwork, safety standards, and professional growth.Preferred less than one year of experience in Long Term Care.
