About the role
The Licensed Practical Nurse (LPN) - Hospice collaborates with the RN Case Manager to develop and implement the hospice plan of care. The LPN communicates with the patient’s provider and the hospice inter-disciplinary team to meet the needs of the patient and family. Responsibilities include providing evidence-based, patient-centered care at the patient’s place of residence, which may be in a private home or facility, using critical thinking and decision-making skills. The LPN ensures thorough hospice documentation with a focus on symptom management and comfort for both the patient and their family at the end of life.
Requirements
- LPN License
- American Heart Association HealthCare Provider BLS (Basic Life Support)
- Valid driver's license
- Excellent interpersonal and communication skills
- Proficiency with Microsoft software and solid computer skills
- Strong analytical and problem-solving skills, with attention to detail
- Provides own transportation and auto insurance, and complies with all driving laws while on duty
Preferred Qualifications
- Experience as an LPN in a healthcare environment, preferably in hospice
- Basic proficiency with Electronic Medical Records and Microsoft applications
Pay
$23.42 - $34.10/hour. Actual pay is based on years of licensure.
