Schedule: Full-time, Monday through Friday, 40 hours per week with some additional after-hours and weekend backup shifts. Coverage area includes Tigard, Tualatin, King City, and Wilsonville.
About the Role
The Licensed Practical Nurse assists in planning, coordination, and care of hospice patients in accordance with the patient’s plan of care. This position provides direct patient care under the supervision of a registered nurse and works collaboratively with the interdisciplinary team to help achieve positive care outcomes.
Responsibilities
- Provide direct patient care as defined in the State Nurse Practice Act following a patient assessment and an approved plan of care initiated by the registered nurse.
- Responsibilities may include:
- Focused/limited nursing visits
- Telephonic triage
- Night/weekend coverage backup rotation as shared among the nursing team
- Other duties assigned by Hospice Leadership
- Collaborate with hospice team members regarding patient care.
- Perform focused/limited nursing assessments, recognize signs and symptoms of end-stage medical conditions/illnesses, and implement planned nursing and medical interventions as delegated by the registered nurse/clinician.
- Perform duties in compliance with Medicare Conditions of Participation.
- Gather and review documentation in electronic medical records, ensuring accuracy, completeness, and compliance with regulations and certification standards.
- Recognize signs and symptoms of common and routine medical conditions/illnesses, distinguishing between abnormal and new/existing, and implement planned nursing and medical interventions as delegated by the registered nurse/clinician.
- Administer medications and treatments as prescribed by the patient’s clinician. Evaluate patient responses to treatments/medications and notify hospice providers and/or registered nurses of concerns.
- Contribute to the development, implementation, or modification of the individualized patient’s plan of care under the direction of the registered nurse/clinician.
- Provide health teaching/education and support to patients and caregivers related to end-of-life care needs.
- Assist with hospice chart auditing, as requested.
Requirements
- Graduate of an accredited LPN training program.
- Minimum one (1) year of experience as a licensed practical nurse in home health, hospice, skilled care facility, memory care unit, or acute care setting (preferably medical/surgical).
- Current unencumbered LPN license in the State of Oregon.
- CPR certification at hire or within 6 months in position.
- Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle.
Skills and Abilities
- Knowledge of Federal, State, and Local regulations, Hospice Medicare Conditions of Participation, and HIPAA privacy rules and regulations.
- Demonstrate nursing abilities and counseling in a hospice setting.
- Practice within the scope of license, certification, or registration, as required by law and regulation.
- General computer skills and working knowledge of Electronic Medical Records systems, MS Office, Outlook, and the internet.
- Excellent interpersonal skills.

- Physical ability to lift, carry, push, pull, climb stairs, bend, and/or pinch small objects for at least 3 hours/day.
- Ability to operate a motorized vehicle in all kinds of weather conditions.
Working Conditions
- Work environments include indoor/office, community, and outdoor settings.
- Member/patient-facing role, both telephonic and in-person.
- Hazards may include physical, ergonomic, and biological hazards.
- Use of general office equipment and/or mobile technology.
- Requires travel outside of the workplace at least weekly; personal vehicle may be used.
- Work location: Community, 5 days/week, member-facing.
- Schedule may occasionally require more than 8 hours per day and may include on-call or after-hours duties.
Pay
Estimated hiring range: $33.98 - $41.53 per hour. Bonus target available.
