Licensed Practical Nurse (LPN) - Hospice

Role: Hospice Licensed Practical Nurse (LPN) Responsibilities Provide compassionate, person-centered end-of-life care in patients' homes and community settings.Deliver skilled nursing under RN/physician supervision, including assessments, pain and symptom management, medication administration, and wound care.Implement individualized care plans and document all care activities in the EMR.Educate patients and families about care procedures and support end-of-life preferences.Collaborate effectively with an interdisciplinary hospice team to ensure quality care outcomes. Qualifications & Skills Licensed Practical Nurse (LPN) license.Strong communication skills, reliability, and respect for diverse backgrounds.Ability to provide empathetic, evidence-based, and outcomes-focused care.
