About the role
Provide 1-on-1 skilled nursing care in patients’ homes, serving pediatric and/or adult populations.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services according to prescribed orders.
- Monitor signs and symptoms to identify physiologic and psychosocial changes.
- Collect, analyze, and document health data and potential nursing diagnoses.
Qualifications
- Licensed as an LPN/LVN in the practicing state.
- Current TB test or Chest X-Ray and BLS certification.
- Preferred at least one year of professional nursing experience.

- Strong assessment, documentation, and patient care skills.
