About the role
Licensed Practical Nurse (LPN) providing one-on-one skilled nursing care in patients' homes.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services following prescribed orders.
- Monitor signs and symptoms indicating physiological or psychosocial changes.
- Collect, analyze, and interpret data from healthcare team to document nursing diagnoses.
Qualifications
- Licensed as an LPN/LVN in the practicing state.
- Current TB or Chest X-Ray and BLS certification.
- Preferred: at least one year of professional nursing experience.
