About the role
Helen Porter Rehabilitation & Nursing is a skilled nursing community located in the peaceful town of Middlebury. We are seeking a full-time LPN to join our team providing exceptional care in a warm and supportive environment.
Responsibilities
- Provide skilled nursing care to residents, including medication administration, wound care, and other treatments as directed by a physician.
- Assist residents with activities of daily living such as bathing, dressing, and eating.
- Participate in interdisciplinary team meetings to discuss resident care plans and coordinate care among nursing staff, therapists, and other healthcare providers.
- Document resident care in accordance with regulatory requirements and facility policies.
- Ensure residents' safety and comfort by maintaining a clean and organized living environment.
- Adhere to infection control protocols and follow all safety guidelines.
Requirements
- A high school diploma or equivalent.
- A current, active LPN license in the state of Vermont.
- At least one year of recent, relevant work experience in a healthcare setting.
- Strong communication skills and ability to interact effectively with residents, families, and healthcare professionals.
- Ability to work flexible hours, including evenings, nights, weekends, and holidays.
Qualifications
- Experience with geriatric care and understanding of the unique needs of older adults.
- Knowledge of basic medical terminology and pharmacology.
- Proficiency in using electronic health records (EHR).
