Licensed Practical Nurse (LPN) - Full-Time PM Shift. Eventide on Eighth Care Center!

Eventide on Eighth is hiring for a Full-Time LPN to work the PM (8 hours) shift (2:30p-11p), with every third weekend and holiday rotation. Eventide will reimburse the fee to obtain a border-state license upon hire for ND nurses. A $2.00 an hour weekend shift differential is added to the base wage.

About the Role

The Charge Nurse ensures that the highest degree of nursing care is provided in accordance with current federal, state, and local standards, guidelines, and regulations, as well as Eventide's philosophy, policies, and procedures.

Responsibilities

  • Assess residents’ needs and health status through physical assessments, observations, and interviews continuously.
  • Provide individual attention, which encourages each resident’s ability to maintain or attain the highest physical, mental, and psychosocial well-being.
  • Observe and follow the residents’ plans of care. Update and make appropriate changes to plans of care as needed. Plans of care include interdisciplinary care plans, physician orders, and C.N.A/NA-R care plans.
  • Administer medications and treatments and monitor both for effectiveness.
  • Provide direct nursing care to residents as needed, in accordance with established nursing policies, procedures, and standards.
  • Comply with rules and regulations as set forth by the Minnesota or North Dakota Department of Health and the Centers for Medicare/Medicaid Services.
  • Provide accurate documentation in the residents’ electronic medical records according to established nursing policies, procedures, and standards.
  • Assist in orientation/training of new team members.
  • Report resident needs, problems, and observations regarding residents’ condition(s) to the Charge Nurse and/or attending physician.
  • Ensure adequate staffing levels for a scheduled shift and correct inadequate staffing appropriately.
  • Monitor the activities and functions of subordinate staff, which may include participating in the disciplinary process.
  • Ensure that work responsibilities of subordinate staff are completed thoroughly and accurately within scheduled shifts and according to established resident care policies, procedures, and standards.
  • Provide information, education, and support for assigned residents and their families.
  • Communicate the needs of residents to resident care staff in the unit, as appropriate.
  • Interact with physicians and other health care facilities as needed, according to the needs of the residents.
  • Continually update nursing skills and knowledge by attending in-services, continuing education programs, and staff meetings.
  • Coordinate and participate in resident care conferences and communicate information effectively to families.
  • Assume responsibility for promptly reporting any incidents or evidence of abuse, neglect, or violation of residents’ rights per policy.
  • Manage electronic timecards daily.
  • Carry out or assist with other tasks in addition to the duties listed in this job description as needed.

Requirements

  • Active MN LPN license or ability to obtain one upon hire.
  • Experience in Long Term Care preferred, but not required.

Pay

Competitive estimated pay range of $29 - $32.48 per hour for LPNs, based on qualifications, experience, and location. Shift premiums and differentials are also available for select positions, paid on employment status and shift worked.
