About the Role
A Licensed Practical Nurse (LPN) in a rehab unit, providing patient care to long-term and post-acute residents.
Responsibilities
- Administer medications accurately and document properly.
- Perform treatments and monitor resident progress per physician orders.
- Ensure complete and concise documentation using computerized records.
- Supervise and support CNAs in delivering personalized resident care.
- Communicate effectively with staff, physicians, and other healthcare team members.
- Transcribe and follow up on physician orders promptly.
- Monitor residents' conditions and report changes timely.
- Complete medication record reviews and MDS documentation.
- Assist with direct ADL care to support CNA efforts.
Requirements
- Graduate from an accredited School of Practical Nursing.
- Current NY licensure and BLS certification.
- Preferred 1 year of experience.
- Strong leadership, organizational, and communication skills.
- Ability to perform medium physical work and assist with resident mobility.
