Licensed Practical Nurse (LPN)

About the role

We are seeking a compassionate nurse to join our dedicated team in a skilled nursing facility. Every day you will provide comfort, dignity, and exceptional care to those who need it most. The primary purpose of this role is to assess residents’ needs, develop individualized care plans, administer nursing care, evaluate nursing care, and supervise nursing assistants and other personnel in the delivery of nursing care in a SNF setting. All supervision and care must align with current federal, state, and local standards, guidelines, and regulations, as well as the community’s policies and procedures, to ensure the highest quality of care at all times.

Responsibilities

• Assess residents’ needs and develop individualized care plans
• Administer nursing care and evaluate its effectiveness
• Supervise nursing assistants and other personnel in the delivery of nursing care
• Follow all health, sanitary, and infection control policies
• Maintain established standards of practice per Nursing Policies and Procedures

Education/Qualifications

• Current, unencumbered LPN license in the state of practice
• Current CPR certification
• Basic computer skills and ability to navigate electronic health record software
