About the Role
Pinnacle Health & Rehab at Canton is seeking a dynamic and highly motivated Licensed Practical Nurse (LPN) to join our clinical team. You will provide high-quality patient care in our 66-bed Assisted Living facility, which holds a consistent 5-star quality rating from the Centers for Medicare & Medicaid Services.
Responsibilities
- Provide direct patient care under the supervision of a Registered Nurse (RN) or other licensed healthcare providers
- Lead and oversee a clinical team, working collaboratively with direct care workers and ancillary departments
- Develop and implement individualized care plans for residents, ensuring a high level of customer satisfaction
- Monitor and report changes in residents' conditions to the RN or other licensed healthcare providers
- Participate in quality improvement initiatives and maintain accurate and up-to-date records
- Collaborate with other healthcare professionals to ensure seamless continuity of care
Requirements
- Active Maine nursing license
- Proof of MMR, varicella, Hep B, and seasonal flu vaccination (Oct-Apr)
- Ability to work every other weekend with on-call responsibilities
- Experience in Assisted Living Care is a plus, but not required. New graduates are encouraged to apply
