About the Role

The Licensed Practical Nurse (LPN) is responsible for providing general nursing care to patients, under the direction of a Registered Nurse and in accordance with physician’s orders and established standards and protocols. This position must integrate company values into daily practice.

Responsibilities

Provides skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse.
Implements orders for medications and treatments as ordered by the physician and in accordance with nursing care plan as written by the RN.
Provides information to the Registered Nurse to assist in the development of an individualized plan of care for each patient.
Participates in evaluation of the effectiveness of interventions in relation to patient responses and seeks guidance in redirection when necessary.
Reports to supervisor on patient care rendered, problems concerning patients’ needs, and resolution to those needs.
Helps identify patient problems and potential risks and contributes to the development of interventions.
Assists in identification of patient’s learning needs and reinforces self-care and other techniques.

Requirements

Maintains regular attendance in keeping with assigned work schedule.
Performs other duties as assigned to support overall effectiveness of the organization.

Qualifications

Minimum Education & Experience : Two years’ experience in inpatient hospital setting, inpatient rehabilitation experience preferred.

Skills

Knowledge of and adherence to current nursing theory and practice and infection prevention standards of practice.
Knowledge of accreditation standards to ensure adherence to all standards set forth by state and accrediting agencies of TJC and CMS.
Knowledge of clinical operations and procedures, and utilizes proper policies, procedures, and guidelines in caring for patients.
