Licensed Practical Nurse (LPN)

About the role

The Licensed Practical Nurse (LPN) cares for a group of residents, assuming responsibility and accountability for their well-being. Nursing is provided through assessment, implementation, and evaluation of the plan of care.

Responsibilities

• Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state, and local standards, guidelines, and regulations.
• Performs required resident assessments, observations, and interventions; reviews care plans to ensure appropriate care.
• Takes and records residents' blood pressure and vital signs; administers catheter care and IV therapy within license limitations; provides respiratory care (e.g., oxygen, nebulizer treatments).
• Charts nurse’s notes in an informative and descriptive manner reflecting care provided and resident’s response.
• Ensures the resident’s environment is free from accidents and hazards; immediately corrects potential safety risks.
• Monitors and directs non-licensed ancillary services to meet professional standards and maintain high-quality resident care.
• Communicates and reports information to residents, families, physicians, guardians, governmental agencies, and facility personnel as required by policy and regulations.
• Ensures all aspects of resident care plans are implemented; recommends care plan changes to the IDT per professional standards.
• Ensures compliance with infection control standards; addresses non-compliance immediately.
• Follows company policies and procedures regarding HIPAA compliance.
• Performs other assigned duties as needed.

Requirements

• Nursing degree or diploma from an accredited college or university.
• Current license to practice as a Licensed Practical Nurse in the state.
• 1 year of experience preferred; long-term care experience preferred.

Physical Requirements

• Walking, reaching, bending, lifting, extended sitting, grasping, and fine hand coordination.
• Ability to distinguish smells and temperatures, with or without mechanical aids.
• Limited potential for exposure to environmental hazards.
• Understanding and adherence to company safety standards and protocols.
