Licensed Practical Nurse (LPN)

Responsibilities

• Provide direct patient care to residents, assisting with activities of daily living under the supervision of a professional nurse
• Assist in implementing individualized treatment plans for each assigned resident
• Supervise Certified Nursing Assistants (CNAs) and participate in their evaluation
• Oversee Falls Program and Pressure Sore Program
• Provide nursing services and conduct/document thorough evaluations of residents’ medical status upon admission and throughout treatment
• Other duties as assigned

Requirements

• Current, unencumbered, active state license to practice as an LPN
• Experience in Skilled Nursing/Rehabilitation facilities preferred
• Compassionate, dedicated, and qualified to perform the job well
