About the role

We are seeking compassionate and dependable Licensed Practical Nurses (LPNs) to provide one-on-one care to individuals in the comfort of their homes throughout the Chester County area. Our team shares available client opportunities with you based on factors such as location, schedule, hours, and care needs, giving you the ability to choose which cases best fit your availability and preferences. Whether you're looking for consistent hours or additional shifts, we offer a variety of open cases.

In this role, you will focus on one individual at a time, getting to know the client and their family while providing consistent support based on their specific needs.

Responsibilities

  • Provide one-on-one nursing care in the client's home following their individualized plan of care.
  • Monitor the client's condition and report changes as appropriate.
  • Administer medications and treatments as ordered and within scope of practice.
  • Assist with activities of daily living and other care needs.
  • Maintain accurate and timely documentation of care provided.
  • Communicate effectively with the client, family, and the 365 team.
  • Follow agency policies, procedures, and applicable nursing standards.
  • Provide compassionate, attentive, and dependable care throughout each scheduled shift.

Requirements

  • Current and active LPN license/certification in the state of Pennsylvania.
  • Attend in-person Orientation at our Downingtown office.
  • Recent physical and CPR certification.
  • Child Abuse Clearances and recent TB test.
  • Two forms of ID (including driver's license).
  • Clinical care skills, including direct patient care, vital sign monitoring, assistance with mobility and ADLs, and basic wound or treatment support within scope of practice.
  • Strong assessment, observation, and documentation abilities.
  • Effective communication and interpersonal skills to collaborate with consumers, families, and multidisciplinary team members.
  • Knowledge of home care best practices, infection control, safety protocols, and relevant state and federal regulations.
  • Reliability, professionalism, and the ability to work independently in the field while following established care plans and agency policies.
  • Basic computer or electronic documentation skills and ability to learn agency systems.
  • Reliable transportation and ability to travel locally to consumer homes as needed.

Preferred Experience

  • G-tube care and feedings
  • G/J tube care and feedings
  • Tracheostomy care
  • Ventilator care
  • Seizure monitoring and precautions

Note: Prior home health, home care, or long-term care experience is preferred but not mandatory.

Pay

$30–$40/hour. Specific pay rates and additional information regarding benefits and eligibility will be discussed during the hiring process.
