LICENSED PRACTICAL NURSE | LONG TERM CARE UNIT 11-7 |FULL-TIME EVENINGS (20822)

Patient Care Duties

• Collects data from patient, family or significant others and records on Initial Data Base form.
• Contributes to the formulation and updating of plan of care and implements plan of care under the direction of the Registered Nurse.
• Gives direct nursing care to assigned patients (e.g., medications, treatments, dressing changes, TPR) according to established standards.
• Recognizes changes in patient condition and responds appropriately, notifying charge nurse as needed.
• Documents pertinent information regarding patient's responses and nursing interventions in patient's medical record.
• Administers medication according to hospital policies.

Caregiver Responsibilities

• Applies principles of infection control and safety in the performance of patient care.
• Communicates effectively with charge nurse and other members of the Health Care Team regarding patient's needs and/or changes in condition.
• Responds appropriately in crisis situations.
• Reports significant unit problems to the R.N.
• Delivers patient care responsibilities with a knowledge of patient growth and development and is appropriate to the ages of the patients served.

Customer Service

• Respects, is flexible, knowledgeable, confident, professional, pleasant, patient, and helpful.
• All responses should be timely, professional, caring, and respectful in accordance with Customer Service Performance expectations.

Departmental Compliance

• Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards.
• Affirms understanding and adherence to the Medical Center’s Code of Conduct.
• Familiarity with the Medical Center’s Mission, Vision, and Values Statements.

Other Duties

• Orients patient and family to new environment; develops rapport with patients, family, and/or significant others.
• Orients LPNs, Nurses’ Aides, and orderlies as required.
• Utilizes the hospital computer system as it pertains to job responsibilities and in accordance with hospital policy.
• Participates on unit committees and in CQI activities as assigned.
• Participates in patient care conferences and in staff development activities as required.
• Assists other personnel as needed.

Qualifications

• Graduation from an accredited school of Practical Nursing or equivalent education as described by the State Education Law.
• Current NJ license as an LPN.
• Successful completion of Progressive Care Orientation Program required.

Skills

• American Heart Association Basic Life Support certification required.
• Clinical nursing skills appropriate for an LPN.
• Good oral and written communication skills.
• Good interpersonal skills.
• Speaks, reads, and writes English to the extent required by the position.
