Position Summary

The Licensed Practical Nurse (LPN) is responsible for overseeing and coordinating medical care services for individuals receiving support through our programs. Working as part of a multidisciplinary team, the LPN provides nursing services from a holistic, person-centered, and values-based perspective while promoting physical health, behavioral health, recovery, and overall wellness.

Essential Responsibilities

  • Clinical Care and Health Monitoring

  • Assess, monitor, and respond to new, ongoing, and emerging health concerns.

  • Conduct health screenings and assessments using validated tools related to depression, trauma, substance use, and identified treatment needs.

  • Maintain and document vital signs, health conditions, and medication side effects.

  • Assess healthcare needs and recommend health-related goals and interventions to support recovery and wellness.

  • Administer intramuscular (IM) injections and other treatments as prescribed.

  • Manage the ordering, delivery, storage, administration, and documentation of medications in accordance with organizational policies and regulatory requirements.

  • Collaborate with individuals and treatment teams to develop strategies that improve medication adherence.

  • Ensure individuals maintain prescribed medication schedules and receive appropriate follow-up care.

  • Care Coordination

  • Coordinate medical care with physicians, prescribers, specialists, primary care providers, and other healthcare professionals.

  • Act as a liaison between consumers, healthcare providers, and community resources.

  • Afford support to individuals in accessing services that promote integrated physical and behavioral healthcare.

  • Support efforts to ensure individuals receive annual physical examinations, dental care, and preventive healthcare services.

  • Recovery-Oriented Services

  • Provide nursing services that support evidence-based practices and recovery-focused treatment.

  • Participate actively in the development and implementation of individualized recovery plans.

  • Support individuals in achieving personal wellness and recovery goals.

  • Deliver culturally responsive services that honor and respect each person's unique identity, background, experiences, and needs.

  • Education and Training

  • Provide health education and wellness information to individuals receiving services.

  • Offer guidance and training to staff regarding common health concerns, medication administration, and healthcare best practices.

  • Promote health literacy and informed decision-making among consumers and team members.

  • Documentation and Compliance

  • Complete assessments, progress notes, treatment contacts, recovery plans, and related documentation in a timely and accurate manner.

  • Utilize collaborative documentation practices.

  • Maintain compliance with organizational standards as well as funding, licensing, accreditation, and regulatory requirements.

  • Crisis Support

  • Participate in a rotating on-call schedule to provide 24-hour emergency and crisis intervention support.

  • Respond appropriately to urgent medical and behavioral health situations.

Qualifications

  • Current Licensed Practical Nurse (LPN) license.

  • Current unrestricted license to practice in the State of Delaware.

  • Valid driver's license.

  • Strong clinical assessment and medication management skills.

  • Ability to work effectively within a multidisciplinary team environment.

  • Excellent communication, problem-solving, and organizational abilities.

Preferred Experience

  • Experience working in behavioral health, community health, recovery-oriented services, or human services settings.

  • Knowledge of trauma-informed care and person-centered practices.

  • Work may occur in community-based settings, residential environments, offices, healthcare facilities, and other service locations.

  • Walk frequently throughout work environments

  • Use hands and fingers to operate equipment and perform nursing tasks

  • Communicate effectively through speaking and listening

  • Visually assess individuals, medications, documentation, and environments

  • Push and pull equipment or materials

  • Perform repetitive motions

  • Reach above and below shoulder level

  • Kneel, stoop, crouch, and crawl when necessary
