Position Summary
The Licensed Practical Nurse (LPN) is responsible for coordinating, monitoring, and responding to the healthcare needs of program participants. This role provides hands-on nursing care, medication management, health assessments, clinical guidance, and ongoing communication with interdisciplinary team members to ensure individuals receive appropriate and timely care.
What You'll Do
- Provide direct nursing care and clinical support to program participants.
- Assess and respond promptly to health concerns, changes in condition, and medical needs.
- Administer medications and treatments in accordance with physician orders, nursing standards, and organizational policies.
- Maintain accurate, complete, and timely medical records.
- Document assessments, treatments, medication administration, and health-related observations.
- Coordinate healthcare services and communicate participant needs, concerns, and updates.
- Support preventative healthcare initiatives and overall wellness goals.
- Participate in health-related meetings, care planning discussions, and team consultations.
Qualifications
- Current Licensed Practical Nurse (LPN) License in good standing.
- High School Diploma or GED equivalent.
- Valid driver's license.
- Current vehicle registration and access to reliable transportation.
- Ability to travel between program locations as needed.
