Position Summary
Abraxas Youth and Family Services is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide quality healthcare services to youth in a residential treatment environment. The role involves supporting client health and wellness through direct care, medication administration, health education, documentation, and emergency medical response.
Key Responsibilities
- Assist with the delivery of client medical care under the supervision of licensed medical professionals.
- Serve as the designated Responsible Health Authority.
- Administer prescribed medications and vaccinations in accordance with physician orders.
- Maintain accurate medical records, health histories, and documentation in compliance with HIPAA regulations.
- Respond to medical emergencies and provide first aid as needed.
- Coordinate referrals to physicians, specialists, and community healthcare providers.
- Participate in on-call coverage for emergency medical situations.
- Attend and contribute to multidisciplinary treatment team meetings.
- Coordinate routine and emergency medical services for clients and staff.
- Provide health education to clients, including hygiene, nutrition, and wellness practices.
- Evaluate the quality and effectiveness of medical services provided.
- Ensure compliance with applicable healthcare licensing and regulatory requirements.
Qualifications
- Required: Current Licensed Practical Nurse (LPN) license or Registered Nurse (RN) license in the State of Illinois.
- PREFERRED SKILLS & ABILITIES: Strong leadership, decision-making, and problem-solving skills; ability to manage challenging or resistant behaviors in a professional manner; effective verbal and written communication skills; strong organizational and time management abilities; ability to work collaboratively within a multidisciplinary team; basic computer proficiency and experience with standard healthcare software systems.
Additional Requirements
- Must be at least 21 years of age.
- Valid driver's license required.
- Successful completion of physical examination, TB testing, and drug screening.
- Ability to pass federal, state, and organizational background checks.
- Satisfactory completion of employment verification, reference checks, and driving record review.
- Ability to safely manage and respond to aggressive behavior when necessary.
