Licensed Practical Nurse - Hospice

About the role

We are seeking an Licensed Practical Nurse (LPN) to provide compassionate, patient-centered hospice care. This role requires a dedicated, compassionate professional committed to providing dignity and strength to hospice patients and their families.

Responsibilities

• Delivering direct patient care
• Coordinating services with the healthcare team
• Completing accurate documentation in compliance with policies and regulations
• Following personalized care plans developed by Registered Nurses
• Accepting and transcribing physician orders
• Sharing clinical progress notes with team members
• Participating in interdisciplinary team meetings

Qualifications

• Successful completion of nursing education from an approved school
• Current LPN licensure
• At least one year of clinical experience, preferably in medical/surgical settings

Skills

• Ability to independently organize work and deliver effective care
• Excellent communication skills (oral and written)
• CPR certification
