Licensed Practical Nurse - Hospice

About the role

Sentara Hospice is seeking a Licensed Practical Nurse (LPN) to provide direct, one‑on‑one care to patients in their homes. The LPN monitors patient health, administers treatments, manages medications, and offers essential support to patients with various medical needs. Collaboration with Registered Nurses (RNs) and physicians is required to deliver high‑quality, compassionate care that improves patient well‑being.

Responsibilities

• Provide direct, one‑on‑one home health care to patients.
• Monitor patient health status and report changes.
• Administer prescribed treatments and manage medications.
• Offer essential support and education to patients and families.
• Collaborate with RNs, physicians, and other healthcare professionals to coordinate care.

Requirements

• Trade school graduate for Nursing (required).
• Current LPN license in Virginia (required).
• Basic Life Support (BLS) certification (required within 90 days).
• Minimum 2 years of nursing experience (required).
• Prior hospice experience (strongly preferred).

Pay

Base pay range for full‑time employment is $28.57 – $43.04 hourly. Additional compensation may include shift differentials, standby/on‑call pay, overtime, shift premiums, extra‑shift incentives, or bonus opportunities. Compensation varies based on qualifications, experience, location, market conditions, and business needs.

Schedule

Full‑time (40 hours/week), Night Shift – Monday through Friday, 12:00 AM – 8:30 AM.
