Licensed Practical Nurse: Home Health - South Fulton, GA

About the role

Visiting Nurse Health System (Visiting Nurse) is seeking a Licensed Practical Nurse (LPN) to join our Home Health team. We are looking for a skilled nurse who is also a calm, compassionate, and empathetic presence, caring for our patients. The position is headquartered out of our Hartsfield office, near the airport, and requires working Monday through Friday across Decatur, GA. This is a non-exempt position.

Responsibilities

• Provide nursing care, emotional support, patient and family education regarding disease process, self-care techniques, prevention strategies, clinical nursing observations, and documentation for Home Health patients.
• Provide nursing services as ordered by the physician and under the direction of the RN.
• Report subjective and objective patient findings to the RN.
• Under the direction of the RN, obtain the physician’s orders and ensure orders exist for each visit made.
• Provide necessary skills to report changes appropriately; document pertinent information and care rendered to the patient to ensure continuity of care.
• Demonstrate knowledge of infection control standards and techniques as per VNHS policy and procedures, and consistently practice these techniques.

Requirements

• Graduate from an accredited school of nursing or program as an LPN.
• Completion of an IV training program or certification in IV therapy.
• Experienced in advanced wound therapies.
• Current license by the Georgia State Board of Nursing.
• Minimum of one to two (1-2) years’ experience in a Medicare-certified home health agency is preferred.
• Minimum of two (2) years as an LPN.
• Current valid Driver’s License, proof of auto insurance, and a reliable vehicle.

$62,338 - $93,507
